Use this form to request reimbursement for your personal Keyset account. To qualify for reimbursement, your request must meet the following criteria:

- Your request must be made within 364 days of the start of your personal subscription.

- This request must result in the transfer of your personal account to a district/school/university organization account*. All prior keyboards and information, including log-in information, will remain the same.

*Important: Your district/school/university must qualify for accounts added via organization codes in order to use this form. In most cases, this requires your district to acquire 10 or more Keyset for Education Licenses and to have one dedicated to you.
